United Way Partner Agency Reports are due at the end of each quarter. See the schedule below:

1st Quarter Due October 20
2nd Quarter Due January 20
3rd Quarter Due April 20
4th Quarter + Annual Due July 20

If extensions are needed, please send a written request to priscilla@unitedwaymsjc.org & janie@unitedwaymsjc.org before the deadline.
Late and/or incomplete reports will result in withheld monthly allocations with additional penalties as outlined in the partner packet.

Measurable Outcomes:
The agency will submit outcome measurement reports via a secured portal on our website monthly, or on a quarterly basis, as requested in the allocation letter or subsequent communication. The United Way shall withhold allocation checks if the report is incomplete and/or not received on time. If a report is late, the monthly allocation of grant funds will be held until after the next scheduled payment date. Funds will not be released until the report is received. If a report is more than 10 days late, a penalty of 10% will be subtracted from
monthly allocation and will increase 5% every 5 days. If late filing for next/other reports becomes a habitual issue, penalties will be applied to the annual allocation.
IT IS THE RESPONSIBILITY OF THE AGENCY TO REMEMBER TO SUBMIT REPORTS IN A TIMELYMANNER.
